                                                         SADPP Community Health Nurse

  Flemingdon Health Centre (FHC) is a registered charity and an incorporated not-for-profit Community Health Centre (CHC). Based in Toronto, we provide a range of health related services based on the social determinants of health and community engagement models. We are primarily funded through the Ministry of Health/Toronto Central Local Health Integration Network (TC-LHIN).

The South Asian Diabetes Prevention Program (SADPP) was started by FHC in 2009 with the overall goal of providing local South Asian communities with equitable access to services by offering language-specific and culturally-relevant diabetes prevention services. SADPP is funded as part of the Ontario Diabetes Strategy.

We are seeking a Community Health Nurse to work collaboratively within the SADPP multi-disciplinary team. This role is designed to engage members of the South Asian community and work to successfully connect them to Primary Care Providers, Diabetes Education and Management Programs, and Health Promotion services. Your contributions will make a difference!

Employment terms: 1 Year (Maternity Leave) (28 hours/week, with the expectation of working some evenings and weekends)

Reporting to: Program Coordinator, SADPP

Salary Range: $44,009.60 to $52,864.00, (28 hours per week)

Expected Start date: April 30 2018.

Location:  Flemingdon Health Centre, 10 Gateway Blvd. Toronto, ON M3C 3A1 and/or offsite program locations

Qualifications, Skills and Proficiencies:

  • Certification as a Registered Nurse in the Province of Ontario
  • Member or eligible for membership with the College of Nurses of Ontario
  • Strong background in research and academic writing, with experience in supporting and/or submitting articles to peer-reviewed journals
  • Experience with research, clinical trials and knowledge of Good Clinical Practice Guidelines, preferred.
  • Sensitivity and knowledge of the needs of multicultural, multiracial communities
  •  Demonstrated  experience or certification in adult-education and behaviour change management
  • Solid communication, public speaking, adult group facilitation and adult education
  • experience
  • Must be skilled in disease prevention, with good understanding of role of health promotion
  • Experience working with a multidisciplinary team where both scope of practice and subject-matter expertise are highly valued
  • Ability to work collaboratively with other sectors in order to address the social and
  • environmental factors that inhibit health and well-being relating to Diabetes
  • Ability to work in non-clinical settings, work some evenings and weekends, and ability to travel offsite are requirements of the position 
  • Certification in as Diabetes Educator (CDE) or working towards completing the designation is an asset
  • Superb organizational skills and exceptionally strong team-work skills
  • Ability to work autonomously, self-directed and able to cope with multiple priorities at one time.
  • Familiarity with Microsoft Office, especially PowerPoint, Excel and Access
  • Valid driver’s license is required and access to a vehicle is an asset. Frequent travel on a regular basis would be required to join team at mobile clinics and events. Most travel would be within the Greater Toronto Area.


Responsibilities include:

  • Plan, deliver, and evaluate diabetes prevention programs and community engagement initiatives for the South Asian communities in collaboration with the program team members. Programs may include: group teaching, support groups, community physical activity, stress management initiatives, and other projects
  • Participate in conducting mass screenings for South Asians not yet living with diabetes
  • Utilize innovative approaches to patient adult-education during an in-depth initial session and ongoing follow-up and support. Improve patient outcomes by using evidenced-based approaches; and motivational communication to assist program participants to make realistic short and long term goals
  • Disseminate comprehensive nursing knowledge and skill using adult learning principles while educating colleagues, peer and staff
  • Develop and evaluate educational resources, conduct environmental scan for existing tools/resources and identify potential collaborations
  • Participate in developing clinical outreach to primary care providers and community engagement activities in the South Asian communities
  • Promote, develop and sustain on-going relationships with primary care providers within the catchment to elevate the SADPP’s community profile and uniqueness of program model
  • Take ownership of client-referrals to services to ensure active enrolment/attachment  and meeting emerging health needs of South Asian communities
  • Meet monthly performance targets of the program
  • Demonstrate innovation and creativity to clinically outreach with primary care providers in Toronto
  • Participate in the development and on-going evaluation of the program
  • Participate in team meetings and centre-wide initiatives
  • Perform administrative tasks, maintain accurate records and write monthly reports
