Title: Intake & Client Navigator 

Employment Terms: Contract-10 months with possibility of extension (35 hours/week) 

Salary Range: $24.43/hr-29.33/hr. (In addition, 4% vacation pay)  

Expected Start Date: Mid-August 2018 

Reporting To: HATP Manager-Community Engagement & Wellness  

Location: Thorncliffe Park Public School, Health Access Thorncliffe Park (HATP) sites and Partner locations in Thorncliffe Park 

About HATP: Heath Access Thorncliffe Park (HATP) is committed to providing coordinated health & Wellness services to the Thorncliffe Park community. It is an innovative partnership between Flemingdon Health Centre (FHC) and TNO-The Neighbourhood Organization (TNO) funded by the TC-LHIN. HATP is a community driven initiative aimed to enhance interdisciplinary comprehensive primary health care, improve access to health, social and community services in Thorncliffe Park.

Note: This is a HATP-TNO position.

Position Summary: The Pediatric Health Initiative – Intake & Client Navigator at the HATP School Based Clinic is designed to assist parents, patients, and caregivers with:

  • Referrals to specialists and other care providers
  • Communication between the child’s primary care, service provider and sub-specialty physicians
  • Support for parental concerns
  • Problem solving to promote the patients’ well being

Additionally, the Intake & Client Navigator will work as part of a team with MDs/NPs in direct clinical care, and offer patients a more direct portal of entry to having needs met at the School-based Clinic. The school-based clinic team should be accessible, family-centred, continuous, comprehensive, coordinated, compassionate, and culturally effective.

Responsibilities

  • First point of contact to the School-based Pediatric Clinic, facilitating intake and assessment of primary care needs and child and family as necessary
  • Maintain regular contact with referral sources; Take referrals and assign to appropriate services within the Pediatric clinic, HATP and beyond
  • Attend and help coordinate team and family meetings
  • Attend all the Individual Treatment Plan meeting working groups when needed
  • Works in collaboration with the HATP team, local service providers and community organization
  • Provides administrative support for the interdisciplinary health services team
  • Establish and maintains strong links with the education and administrative staff of Thorncliffe Park Public School and Frasier Mustard with HATP team
  • Conducts outreach and parent introduction meetings to the School-Based Clinic and HATP activities
  • Organizes public education programs dealing with developmental issues, e.g. autism support, and behavioral management in coordination with HATP Community Health Workers
  • Maintain the registry of children with special health care needs
  • Pre-visit planning in coordination with the HATP Holistic intake and navigation team
  • Facilitate communication between families, HATP team, community services and health care providers
  • Documenting all care provided in the patient’s record in EMR
  • Assist with the evaluation of the quality of care and services provided, identifying any areas that need improvement and making the appropriate recommendations to address any quality deficiencies 
  • Assist in addressing any patient issues or complaints related to the services received
  • Participate in performance and quality improvement activities
  • Provide case management and care coordination for complex clients as needed
  • Maintain an adequate inventory of clinical supplies, equipment and assist with adherence to the budget, cost containment and product procurement as needed

 Skills and Qualifications 

  • Bachelor’s degree in Life sciences, Health or Social Science with 2-3 years of  related work experience Or
  • Combination of equivalent education and experience in a health care environment.
  • Excellent computer skills including Microsoft Office Suite and experience in using an Electronic Medical Records (EMR) system preferably Nightingale On-Demand
  • Demonstrated commitment to client service and satisfaction
  • Excellent interpersonal and listening skills
  • Strong communication, organizational, time management and problem-solving skills
  • Strong judgment and decision-making skills
  • Ability to remain calm and composed when dealing with difficult people or situations
  • Ability to work efficiently in a high-pressured environment with simultaneous demands
  • Adheres to confidentiality policy for sensitive patient data and information
  • Demonstrated ability to work in a team environment and collaborate with others in assisting with the delivery of services
  • Second language, relevant to catchment area populations, is an asset 

Hours of Work: Regular work week will be 35 hours with some flexibility to work in evenings and weekends.
