– apply today to join our team The Patient Population As a Physiotherapist in the Interdisciplinary Long Term Care Practice Team, you will see tenants living in Assisted Living Care homes in Vancouver with a variety of acute episodic or chronic conditions.

You will participate in developing and evaluating this new service delivery model along with your occupational therapy colleagues on the team.

The Nature of your Work You will work in partnership with the client and family to establish goals that are safe, realistic and reasonable by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning physiotherapy and other services for the client in an inter-professional team model in partnership with the client, family, primary care provider and other care providers.

You will assess service eligibility, facilitate the development, implementation and adaptation of a care plan, deliver direct care and provide care management to assist clients to manage their own care and navigate through the various services available within the community setting.

You will work in consultation with other system partners and incorporates current evidence into practice.

This position offers flexibility in terms of work and the resources to work remotely to support both virtual visits and increased face to face clinical time when appropriate, as well as to reduce travel time across Vancouver.

QUALIFICATIONS: Education & Experience Current full registration with the College of Physical Therapists of British Columbia (CPTBC).

Eligible for membership in the Physiotherapy Association of British Columbia (PABC).

Two (2) years’ recent related clinical experience in Home Health and/or in an Assisted Living setting, or an equivalent combination of education, training and experience.

Valid BC Driver’s License and access to a personal vehicle for business-related purposes.

Knowledge & Abilities Basic knowledge of evidence-based clinical physiotherapy practice related to the designated program/area within a client and family-centred model of care.

Broad knowledge of the CPTBC standards of practice and guidelines for clinical practitioners.

Broad knowledge of provincial acts and regulations related to home and community care.

Knowledge of physiotherapy standardized tests and measures.

Knowledge of acute, chronic disease, palliative and health management /self-management support.

Demonstrated ability to teach/coach and empower clients to assume control over their health.

Knowledge of community resources available for client/family support and related health services.

Knowledge of transitions through the continuum of care.

Demonstrated ability to communicate effectively with co-workers and clients both one-on-one and in groups through verbal, non-verbal, written, electronic and other means.

Demonstrated ability to work independently and in collaboration with others as an effective team member.

Demonstrated ability to establish workload priorities in collaboration with others in a complex health care environment.

Broad knowledge of other health care disciplines and their role in client care.

Demonstrated ability to employ effective conflict resolution and reconciliation approaches and techniques.

Demonstrated ability to work in a dynamic environment with changing priorities.

Ability to operate related equipment including applicable software applications.

Basic knowledge of adult education principles, methods and tools.

Basic knowledge of research process and methodology.

Physical ability to perform the duties of the position.
